*** Phase II of the HPS Restart Questionnaire ***

HPS is exploring the possibility of creating a PHASE II of our Restart Plan. As such, please fill out the following information if you would be interested in having your child’s status (Remote-Only/Hybrid) changed to EVERYDAY IN SCHOOL LEARNING. If you want your child to remain remote-only or stay with the weekly hybrid plan then you will not need to fill in this form.
